"Rheumatism" is a loose layman term, often referring to vague aches and pains of indeterminate causes, and fluctuates with the weather.
In western medicine, the more precise term is "Soft Tissue Rheumatism", referring to pain arising from soft tissue outside of the joint (which would be called "arthritis"), such as tendons, ligaments, muscles and nerves. The causes include trauma, sprains, repetitive strain and nerve entrapment. Once the cause is identified, treatment can be curative.
Often, rest and physiotherapy deliver relief. Do what is comfortable for you, be it keeping warm, or icing the injured part. Consult your doctor if not better.
Pain at rest, worse in the morning, and improves with activity suggests inflammatory pain. Given his age, and coupled with history of uveitis, Ankylosing Spondylitis is most likely. He needs early treatment to prevent visual loss and disability from joint damage. He most likely will need treatment with a biologic anti-TNF.
When the Lupus disease is active, it is not advisable to get pregnant. This will increase the risk for miscarriage as well as disease worsening during pregnancy. Lupus may be associated with the Anti-Phospholipid Syndrome, which can cause recurrent miscarriages and blood clots. There is now very effective treatments to bring Lupus into remission, making it safe for patients to get pregnant, have a safe pregnancy, and lead a normal life.
The treatment is not working well enough, if at all. You should feedback to the same doctor, or seek a second opinion.
You will need to schedule an appointment to see a rheumatologist for further evaluation and tests.
